Im gonna guess its Meat's soul/Meatness/drive (as caryl said) and he is with Jim ready to face down what ever they have ahead.

Part of me thinks the cover is as much metaphorical as it is literal. Given this is Meat's last one could view the cover as Jim and Meat at the end of the road looking back and preparing to face their own legacy before they move on to whatever lies ahead.

Edit: Eventually Jim & Meat opened it, and all the evils flew out, leaving only "Hope" inside once they had closed it again.
